Admissions Forms

The following documents should be sent directly to Saint Joseph Catholic Parish School by mail, fax, or scan.

1. Copy of Birth Certificate
2. Copy of Baptism Certificate
3. Release of Records and School Recommendation Form – to be completed by Current Teacher and sent directly from them
   • Pre-Kindergarden Recommendation Form
   • Kindergarten - 1st Grade Recommendation Form
   • 2nd - 6th Grade Recommendation Form
4. Medical Forms -
   • Certificate of Immunization (Form 3231)
   • Certificate of Vision, Hearing, Dental, and Nutrition Screening (Form 3300)
   • Physical Form – to be completed by Physician
   • Medication Permission Form – If you would like our nurse to administer medication to your child, this form must be completed by your physician.
5. Parish Verification Form – to be completed by your current parish priest to qualify for Catholic tuition.
